1. profesjonalne zdjęcie śpiewaka(ów) i pianisty(ów) oraz

2. kopię dowodu osobistego

3. potwierdzenie wpłaty opłaty za uczestnictwo

4. profesjonalna biografia artystyczna śpiewaka i pianisty

e-mailem na adres liedcompetition@gmail.com. Przesyłając formularze, muzycy akceptują regulamin konkursu.

Uwaga: Uczestnicy, którzy mają trudności z wypełnieniem formularzy, mogą pobrać formularz w formacie PDF, podpisać go i wysłać na podany powyżej adres e-mail.
